New feature: have CVD's size filters match T&S

I have Cumulative Volume Delta configured to display small, medium and large trade volume deltas, but CVD doesn't aggregate trades like T&S does.  It simply reports individual trades.  So setting CVD to show large, medium and small trade CVD doesn't really show much of anything.
